The events of Seveso, Bhopal, Mississauga and Sandoz are very briefly described. However, most of the chapter is devoted to a description of the events at DuPont’s Belle factory in West Virginia in 2010 when, during a period of 33 h, a plant with a very good safety record had three significant accidents, one of which was fatal. In particular, the fatal accident occurred when an employee was splashed with phosgene following a hose rupture. (i) The hose should have been replaced every month, but the work instruction on the maintenance database had been accidentally deleted several years earlier. (ii) There was also evidence of “corporate memory fade” caused by a large number of long-served employees who had reached retirement age. Both of these issues are generic issues for any hazardous industry anywhere.
